At Esra Health, we help individuals and families across the country navigate Medicare every year. One question we hear more than any other is: “Do I really need to sign up right when I turn 65?” The short answer is yes—if you want to protect your wallet, your coverage, and your peace of mind.
That critical window is called your Initial Enrollment Period (IEP), and missing it can cost you hundreds or even thousands of dollars over time. Read more to learn all about the importance of your Initial Enrollment Period.
What Is the Medicare Initial Enrollment Period?
Your IEP is a 7-month window that begins 3 months before the month you turn 65, includes your birthday month, and ends 3 months after.
For example, if your 65th birthday is in July:
IEP starts April 1
IEP ends October 31
During this time, you can enroll in Original Medicare (Part A & Part B), add a Medicare Advantage (Part C) plan, choose a prescription drug plan (Part D), or choose a Medigap supplement policy all without penalties or medical underwriting restrictions that apply later.
If you’re already receiving Social Security benefits, you may be automatically enrolled in Parts A and B. But even then, it’s wise to review your options, because automatic enrollment doesn’t automatically include the best prescription coverage or a Medicare Advantage plan that might better suit your needs.
Why Enrolling During Your IEP Matters
Enrolling on time gives you three major advantages:
No Late Enrollment Penalties Miss your IEP and you’ll face permanent penalties on Part B (and possibly Part D). The Part B penalty is 10% of the standard premium for every full 12-month period you delayed—added to your monthly premium for the rest of your life. Those extra costs never go away.
Immediate Coverage Without Gaps or Waiting Periods Enrolling during your IEP means your coverage can start as early as the first day of your birthday month (depending on when you apply). You avoid gaps that could leave you exposed to high medical bills.
Guaranteed Issue Rights for Medigap Policies During the six-month Medigap Open Enrollment Period that begins the month you turn 65 and enroll in Part B, insurance companies must sell you any Medigap policy they offer without medical underwriting or higher rates based on pre-existing conditions. After that window closes, you could be denied or charged more.
In short, your IEP is the one time Medicare makes it easiest—and cheapest—to get the coverage you need.
Why You Should Work With a Knowledgeable Medicare Broker
Medicare isn’t one-size-fits-all. There are dozens of plans in most areas that offer different networks, drug formularies, premiums, and out-of-pocket costs. Trying to compare them on your own can feel overwhelming.
That’s where a licensed, independent Medicare broker comes in. At Esra Health, our team:
Compares every plan available in your ZIP code (not just a few popular ones)
Reviews your current doctors, medications, and health needs to find the true best fit
Explains Original Medicare + Medigap versus Medicare Advantage in plain English
Helps you avoid common pitfalls like unknowingly losing employer retiree coverage or triggering Part D penalties
Stays with you year after year for annual reviews and plan changes
Unlike captive agents who only sell one company’s plans, independent brokers like ours are free to shop the entire marketplace on your behalf. And best of all—our services cost you nothing. We’re compensated by the insurance carriers, not by you.
